Carcinoma of the stomach is one of the most prevalent malignancy in Korea and has nonspecific, symptom and sign, also has poor prognosis(31~0.7%). However, the prognosis of the early gastric cancer(EGC) has been reported excellent (95%) and this
is
why
emphasize the early diagnosis and treatment of this grave proble .
This report deals with the clinical analysis of 39 cases of early gastric cancer which wer confirmed by surgery and histopathological examination during the period of 6 years from January 1988 to December 1993 at Department of General Surgery,
Seoul
Adventist Haspital.
@ES The results were as follows:
@EN 1) The incidence of EGC was 17.3% of all resected gastric cancer. The ratio of male to female was 3.3 : 1 nd the most prevalent age group was 5th decade(51.3%) and the mean age was 52.3 years.
2) The mst frequent complaint was epigastric pain and discomfort and duration of symptom was the most 1 year(71.8%).
3) The preoperative diagnostic rate was 33.3% by UGI X-ray and 57.1% by endoscopy with biopsy.
4) The most common site of cancer was the lesser curvature of lower one third(25.6%) and the frequent sites of lymph node metastasis were lower one third(19.0%) and post wall(42.8%).
5) The most common size of lesions was 1.1~2cm(46.2%). The larger of lesion, the greater incidence of invasion to submucosa was noted, but no related to lymph mode metastasis rate.
6) The most common histological type was poorly differeatiated type(20%) and the poorer of differentiation, the greater incidence of invasion to submucosa was noted.
7) The most common type of lesion was IIb+IIc(38.5) and except I, IIa, all other type were invased to submucosa and the depressed & flat type(IIc+III, III, Iib+IIc) were metastasized to lymph node.
8) The rate of lymph node metastasis was 0% in mucosa, 20.7% in submucosa and 15.4% in total.
9) Follow up studies were possible in 28 cases but among them, death and recurrence were not found.
